This vacation is valid for two round trip airline tickets from an approved point of origin and three days and two nights (double occupancy) of hotel accommodations in either Las Vegas, NV, Orlando, FL, San Diego, CA, San Francisco, CA or Reno, NV. No other costs or fees of any nature shall be paid by Unified Financial Group, Inc. or its affiliates.
This vacation is valid for two adults. There is an additional cost per day for any additional guests, and this cost shall not be borne by Unified Financial Group, Inc. or its affiliates. The vacation does not include the costs of food, beverages, gratuities, ground transportation, or any incidental expenses. This vacation is non-transferable and cannot be sold or redeemed for cash, nor shall any rebates, refunds or credits be issued for any unused time, days or space. This vacation is not valid for use with group travel. Group travel is defined as tow or more families traveling or meeting at the same destination on the same arrival date. This certificate cannot be used in conjunction with any other travel package.
This package is not valid for employees or relatives of Unified Financial Group, Inc, its affiliates, or any participating merchant. Accommodations are subject to availability of allotted space. During holiday, peak travel periods, a surcharge may apply. Unified Financial Group, Inc. and its affiliates reserve the right to substitute alternate destinations or travel dates during periods of heavy travel including, but not limited to, peak travel periods and holidays. This vacation is valid for one year and travel must be completed before the expiration date on the registration form. This vacation is valid for Monday, Tuesday or Wednesday departure only.
The enclosed form must be filled out in its entirety and sent to the address on the activation form. Only original and completely filled out registration forms will be accepted. Once your registration has been received you will be sent correspondence on how to finalize your booking. A customer service representative will be in contact with you approximately 30 days prior to your travel date to finalize booking. When your vacation is confirmed you will be mailed a written confirmation with directions and check in times. Do not leave for your vacation until you have received written confirmation of your reservation. Please not while advance travel dates are recommended, confirmations are not issued more than 90 days prior to travel dates.
Should you need to cancel your reservation prior to it being booked, written notice must be received 30 days prior to your travel date. Any changes/cancellations will result in penalty fees and purchase price of product. This travel certificate must be authorized and paid in full by the merchant from who you received it in order for Travel America to book your vacation.
Unified Financial Group, Inc., its affiliates and Travel America, Inc. do not assume any liability or responsibility for any damage, expense or inconvenience, nor any loss, injury, death or damage to any person or property at any hotel or upon any aircraft or ship, or in transit between destinations, or any elay or cancellation due to any cause.
